Gift Form
In order to top up your Donor Advised Fund or to donate directly to the charity of your choice with either cash or securities in-kind, Benefaction requires that you complete a Gift Form. If you are donating gifts of securities, your broker will likely require that you complete a stock transfer form as well. Please forward completed Gift Forms to Benefaction Foundation.
